O2 fails to boot, Amber light with keyboard attached
Anybody seen this situation. I have a O2 R5K. with the keyboard plugged and power applied. system attempts to start, I heard disk activity but it get stuck with a amber light showing on the front. 

with the keyboard unplugged and power reapplied to the system, it will give a green light give the piano tune. message from the console port say keyboard is missing check the cable, system stops at maintenance menu. I give command to boot and reattach the key board. once it is up the keyboard works fine, I can log in. I run the confidence test on the Keyboard, works great no errors. 

I give command to restart the system, on the way down console message gives:
  "Waiting for 'Asynch. rpc req handler/server' to die.  
   Waiting for 'Asynch. event hander' to die." 

front light goes amber and system hangs. 

I reset follow same procedure to get to the maintenance menu and run diags, all pass. 

Just was wondering if anyone else has experience it and or have ideas for me to try and further pin done the problem. 

the Keyboard is know good work on other SGI gear I have and other keyboards give the same results.

Any ideas or feedback is appreciated.

What occurs to me is that by default, the firmware uses the graphical console when it detects a keyboard, and the serial console when it does not.
So your problem when booting with an attached keyboard could be a graphics problem.
Can you try running the diagnostics (ide) to test the graphics subsystem?

The Graphics seem to be good, It passes the IDE qnd_gfx_test ok, and when fully booted passes all confidence test. perhaps there is another graphics command I'm missing?

seems strange but while in the prom, I issued a resetenv and the system now boots normally?? LOl.

No, battery is good, just thought there might be something Hinky in the environment. something like in the O200 and above where they disable components (memory, cpu(s)) when a problem is detected. I just did a resetenv then a reset, it rebooted up to user interface with keyboard attached. strange, anyway now i'm on to find a power supply or repair place for another O2 I have... always something. LOL
