Indy won't boot

Exclamation  Indy won't boot
I am trying to get my Indy to boot successfully.

When it attempts to start, I get this error:
   

Afterwards it falls back to the main system menu.

Trying "auto" from the system recovery console give me this:
   

I have tried the internal diagnostics program and it gives me this:
   

Hardware wise, the machine is A-Okay. I suspect something might be amiss with the software installation.

So the main question being, is there a way to salvage this or is it time for a fresh IRIX install?

My SGI Hardware:
Indy SGI Indy (1997), R5000 @ 180MHz , 256MB RAM, GR3-XZ graphics, SCSI2SD v5.1 w/ 16GB µSD card, IRIX 6.5.22
O2  SGI O² (1996), R5000 @ 180MHz , 256MB RAM, 2 x 2.0GB SCSI HDD, IRIX 6.5
Other bits: SGI Indycam, Yamaha CRW2100SX-VK External SCSI CD drive, more software than you can poke a stick at.
