SGI O2: New CD-ROM SCSI overflow or underflow
Hi,
Because my original CD drive was broken, I installed a new one (a Yamaha CRW4416S) and put the Installation and Overlays disk in it and started diagnostics. It returned the following:

Checking for Distribution CD-ROM on pci(0)scsi(0)cdrom(4).
Booting IDE from Distribution CD-ROM.
SCSI overflow or underflow on (0,4). SCSI overflow or underflow on (0,4). SCSI ove
rflow or underflow on (0,4). dks0d4s7: SCSI driver error 2
dks0d4s7: Exhausted retries, i/o failed
Cannot load pci(0)scsi(0)cdrom(4)part(7)/stand/ide.IP32.
Problem reading file magic id, err 8 cnt 0.
Unable to execute pci(0)scsi(0)cdrom(4)part(7)/stand?ide.IP32: IO error
Autoboot failed.
pci(0)scsi(0)cdrom(4)part(7)/stand/ide.IP32: IO error.

I'm, not sure what to do. Do I get another CD-ROM or are there drivers for the CD-ROM in there now?

Maybe check the jumper settings on the back of the CD ROM to see if it’s set as drive 4?

Is the drive jumpered for 512byte block operation?

The block size jumper is the one closest to the SCSI connector, and a jumper must be installed.

Parity

It still comes up when I try to access the disk to reformat the hard drive. Is this drive incompatible or does it not matter which drive I use providing it's SCSI?

You have to jumper the one closest to the SCSI connector, this sets the block size to 512bytes, which is required to properly read IRIX install media.

I'm getting the same results as last time. I'm not sure what to do or if my computer is dead.
