Hey guys!
i just got my 13W3 Adapter :) after cleaning every contact and inspecting everything i put Lucy together and it was time to switch her on...
what shall i say, almost instant screen and she was off :) so the adapter works : first good news. and lucy runs (power supply is ok) : another good news.
now theres a question :D cause i don't get into irix gui.
what does this mean (ethernet is clear...)

What keyboard and mouse are you using and what ports are you using for them? (are the keyboard and mouse in their correct PS/2 ports?)

yep it's an octane (upgraded to octane 2 specs i believe) i use a keyboard over ps2 and a mouse over com1 (not working) it feels like theres something stuck in the mouse ps2 port (i will try to get it out with a magnet)

Im wondering if this is't why you're not getting into the IRIX GUI.
The network error that you're seeing is normal if there is no network connected.

the keyboard is a regular pc-keyboard (it works -> i can type stuff)

Im not 100% sure, as I haven't experimented with this, but I don't know whether you can get into the IRIX GUI without a mouse being present.
What do you see once IRIX has loaded?
If I remember correctly, in the past, I think I had an issue with my O2, where a particular mouse that I was trying to use with the machine didn't work, and I couldn't get into the IRIX GUI.
Try to clear that mouse PS/2 port and plug the mouse in.

Try to get that mouse plugged in the PS/2 port and take the mouse out of the serial port.
Take another picture as IRIX loads, once you've done this, as the warnings concerning the keyboard and the stuck port aren't normal.
