Looking for advice, I have a second Octane that I've cobbled together with an EMXI and a Digital Video board running 6.5.22. I've followed the posts listed below, I must be missing a step as the digital video board will not load.

It looks like the requisite packages are there on your installation. The
hinv shows the presence of the Galileo board, but I'm not seeing any other entries under the EMXI for it. There should be an entry there according to the manual (page 35) here:
https://irix7.com/techpubs/007-3466-001.pdf. I'll also try to get a
hinv from my own Octane for comparison when I can.
Pages 35-36 suggest a few troubleshooting steps, including verifying that the boards are firmly in place in the Octane and that the flex cables are all connected firmly. I'd run through those steps and see if it makes a difference. If you see any dirt/dust on the compression connectors, official recommendation from SGI was to use nothing more than some light canned air duster, as stated here:

If there's no difference after all that, let me know and we'll see what other troubleshooting steps we can take.

Thanks for the response! I've reseated the ribbon cables along by doing so the whole tray again. I'll pick up a can of compressed air today and see if the inside connectors need to be cleaned; however, the caps on the boards look immaculate.
Once I obtain a can of compressed air I'll do this all over again and see what the outcome is. I've looked again to ensure I didn't miss anything, and hinv and gfxinfo haven't changed. I don't see the entries such as what the manual states on page 35, which is odd as it does have the entry in hinv.

Happy to help! I was booting up my Octane to get a
hinv and realized that I need to do the installation again myself, as I reinstalled IRIX recently. I'll run through the process again and make sure I can get it to work. I'll document the steps and pull a
hinv when I'm done.

I'd like to see the hinv, as I"m currently stumped.
I've blown it out the chassis, reseated the ribbon cables which appear to not be damaged in any way shape or form and still there are no changes to hinv or gfxinfo. The only remaining piece that hasn't been done was to pull the two cards apart and reseat the ribbon that is in the middle of the cards.

I just got it working again on my Octane. I had some issues with video daemon errors, myself, even after installing
dmedia,
impactdm, and
impactvideo. Here are the steps I'd recommend trying next:
1. Make sure that
videod is running. On my base IRIX install,
videod was set to "off" in the
chkconfig list. Check that first and see if the Video Panel will load after that. If not...
2. Uninstall
impactdm and
impactvideo. I used the Software Manager, but
inst works, too. Reboot the system.
3. Reinstall
impactdm and
impactvideo. Reboot the system again.
4. Profit, hopefully. I was able to get the Video Panel to open and the Digital Video Option working at this point.

I reinstalled impactdm and impactvideo 4 times this morning rebooting after each step of uninstalling and reinstalling without any luck. I checked videod is on with chkconfig, the panel never loads.
What I did catch on boot was "unknown device on gio bridge".
I'm running 6.5.22, are you running .30? I'm halfway tempted to wipe the slate clean and install from a new disk to see if maybe my base install is causing the issue.

I am running .30, yeah. I don't imagine that it should make a significant difference, as long as you're using version-appropriate overlays for
impactdm and
impactvideo from overlay disc 2's
unbundled folder.
My IRIX installation was done with Booterizer, using the standard instructions there, plus also installing
dmedia. I suppose there could be something in your base installation causing issues. I'll also note that my installation
doesn't have
evo or
impactcomp installed. I only installed
impactdm and
impactvideo, using octane_iv_6.5.tar in Ian's archive, plus the .30 overlay
unbundled updates.
Hopefully following a similar process will get it working. If not, I suppose there could be an issue with the card, but it is showing up in your
hinv partially. Until the software was in place, that final "Digital Video" line didn't show up for me, either, so I hope it isn't a hardware issue.
Let me know how things go with the re-installation (or if you have any questions in the meantime) and we'll keep troubleshooting after that, if needed.

I loaded .22 onto a new disk through booterizer was having the same issue as before. I then upgraded to .30 and the issue still persists, I only installed octane_iv from Ians site along with the unbundled folder of corresponding items to fix the conflicts.
What gets me is the "unknown device on gio bridge", this happens on either install of .22 and .30. I noticed this during boot up and didn't see this before the EXMI / Digital video was installed.
The only thing that I haven't done is to separate the the cards and see if the ribbon cable needs to be reseated between the digital video boards themselves if that's even possible. To me, it looks as if there is no quick disconnect and that the ribbon is connected firmly. The ribbon does look to be in perfect shape without any visible damage.

The two boards that make up the Digital Video Option are pretty well seated together and I don't think they're intended to come apart. There doesn't seem to be an obvious way to go about it, at any rate. Beyond applying a small amount of spread-out force to the two in order to ensure that everything is seated, I'm not sure there's much else to be done on that front.
The "unknown device on GIO bridge" error suggests to me that maybe the compression connector isn't making perfect contact inside the Octane itself. You could try spraying a bit of air duster inside the XIO bay with the cards removed to clear any dust from the connectors on that side. A full clean of those connectors would require tearing the system down a bit.
Beyond reseating those connectors again and making sure that the boards are in as far as they'll go and firmly locked into place, I'm afraid that I'm running low on ideas. It's possible that there is some sort of hardware issue, although the fact that it does show up at least partially in the
hinv does leave me uncertain.
